Serious Mental Illness

What is Serious Mental Illness?

Some of the most common types of serious mental illness (SMI) are thought disorders and mood disorders that prevent individuals from feeling mentally well and working or living in the community. Clients who experience SMI can have symptoms of losing touch with reality or delusions. New Vista is dedicated to providing clients with SMI the highest level of care by our highly trained clinicians and psychiatrists. We work to keep individuals in the community in which they live by offering services that are evidence-based and specialized to meet the needs of serious mental illnesses.

Specialized Services

Targeted Case Management

Through targeted case management, clients gain access to social, medical, educational and vocational services. New Vista works with clients and caregivers to develop a treatment plan that puts the client at the center of the planning process, including identifying their own strengths and needs. In addition, we work to make referrals and link clients with partner agencies to coordinate services for ongoing needs. We believe strongly our role is to advocate for the client and supporting clients as they advocate for themselves.


Assertive Community Treatment

Assertive Community Treatment (ACT) is an evidence-based practice for individuals with serious mental illness who are most vulnerable to homelessness, incarceration and hospitalization. ACT is a community-based program that offers intensive and highly individualized services. Some of the services provided are: psychiatry, nursing, therapy, case management, housing, supported employment and peer support. ACT often serves individuals who have not made progress in traditional outpatient care and may have had episodes of homelessness, incarceration and hospitalization. ACT is person-centered and focuses on increasing the individual’s ability to meet self-defined goals, increase skills and gain confidence to maintain long term mental health recovery.


DIVERTS

The goal of the DIVERTS program is to create a path for individuals with serious mental illness to live as independently as possible and in the community of their choice. DIVERTS helps individuals living in a personal care home transition to community-based living. Through DIVERTS, the needs of the individual are established and access to services and support to meet those needs are created. Staff work to provide short-term support for individuals transitioning to the community until they are linked to an ongoing treatment team.


iHOPE

iHOPE identifies and supports young people, ages 15 – 30, who are experiencing symptoms of schizophrenia or bipolar disorder with psychosis. iHOPE works with the entire family to help clarify diagnosis and assure access to appropriate services. New Vista partners with local, state and national entities to implement iHOPE using evidence-base practices and emerging research for the best outcomes. Services are available regardless of a client’s ability to pay. Anyone can refer to iHOPE by calling the 24-Hour Helpine 1.800.928.8000.


Supported Employment Services

Intensive employment services are provided to individuals with SMI who may have difficulty finding sustainable work. New Vista uses evidenced-based, Individual Placement and Support model to customize employment plans to meet each individuals needs and goals. We believe any person who wants to work can be employed with thoughtful planning. Individuals are paired with employment specialists provide ongoing support and are part of the treatment team to ensure continuity of services.


The Drop-In Center

The Drop-In Center is a place for adults with mental illness to find the support and resources they need in a caring environment. Peers connect individuals with therapy, employment, housing assistance and case management. Clothing, snacks, access to computers are provided at no cost.

Therapeutic Rehabilitation Program (TRP)

Elevate is a TRP designed to reduce the effects of SMI and improve the quality of life for participants. Services are provided in group settings or on-on-one. Elevate uses psychiatric rehabilitation techniques that focus on learning daily living skills, developing emotional regulation and improving interpersonal skills. Our goal is to help participants elevate their minds and enrich their lives while becoming more engaged in their local communities. Below is a list of our current Elevate locations.
